Today, tens of billions of dollars are spent unproductively on communications among health plans, members, physicians, and patients with chronic conditions. Mailings, faxes, call centers, and online portals have been only marginally effective at reaching providers and patients to inform patient care: The delivery success rate of these communication methods ranges from 2 to 35 percent. Moreover, they cannot deliver information when and where it is most actionable and impactful–when the doctor and patient are together in the exam room.

Built on top of our e-prescribing platform that is used today by thousands of physicians, Prematics Care Communication service provides an easy-to-deploy, mobile, clinical solution for practices seeking better informed care and a path forward to better care coordination.

In addition to basic e-prescribing functionality, our Care Communication service provides a bi-directional electronic flow of information among doctors, patients, pharmacies, pharmacy benefit managers, health plans, and other stakeholders. Until now, this closed information loop has been unavailable.

We intelligently prioritize and deliver messages to physicians and patients where they are actionable and impactful – in the exam room – as well as immediately before and following the visit.

Examples of Care Communication include alerts about patient adherence, formulary compliance, lower-cost alternatives, drug interactions, prior authorization, pay-for-performance clinical protocols and other information that helps close the information gap, lower health care costs, and improve outcomes.
