Prematics CEO Joins Initiative To Advance More Coordinated, Connected Health Care
TransforMED Names Kevin Hutchinson To Inaugural Board Of Managers

Bethesda, MD (May 27, 2008) - Prematics president and chief executive officer, Kevin Hutchinson, has been named an inaugural member of the Board of Managers for TransforMED LLC, a wholly owned subsidiary of the American Academy of Family Physicians (AAFP). TransforMED will provide support and consulting services to primary care physicians and health systems that want to transform their medical practices into patient-centered medical homes.

The TransforMED medical home model combines the relationship-centered values of family medicine with new technologies to improve care quality, efficiency and patient and physician satisfaction. As patient-centered medical homes, primary care practices serve as the focal point of care coordination across all specialties for patients, maintaining central and complete patient records. The model also calls for adoption of information technologies as an essential component to improving diagnostic capabilities and enhancing treatment and outcomes.
